Refer to EAST

To make a referral to EAST, call or fax a referral form to the EAST Team Lead in the individual's county of residence. The contact numbers for each of EAST's counties are below. Include with your fax all relevant assessments and releases of information.

 

The referral form is located below in the referral packet document. The document also includes guidelines for referrals and frequently asked questions for referents.

If the individual you are referring is in an immediate danger to self or others, you will need to refer directly to the local crisis system. The crisis system will refer to EAST when the crsisis resolves.
